Open a Report based off a Forms Display?

How do you get a report to open with the exact data based off of a form? I
have a form with approx 8 fields. I want to have a button that will a report
based off the exact data I am looking at on my form. How is this done. I can
get as far as putting a button on my form to open a report but it does not
show the data based off the form.

Please help!
