One contact is appearing multiple times in Outlook Address Book.

In Outlook 2003 my contacts appear as a single entry when viewed thru
contacts. If I click on the To... button in an email and attempt to pick a
contact from there the contacts appear multiple times. Once with the last
name, first name, once with the email address, once with the business fax.
If I delete one of these the contact information is gone completely.

Why would it be otherwise?
Think about it. Users need to be able to select which electronic address to
use for each Contact. Just selecting the Contact won't help because most
Contacts have more than one electronic address.
 
But...that is not the behavior that I want. Is there a way to only see the
one entry per contact and maintain the rest of the contact information?

Sure. Create Contacts that have only one electronic address.
Outlook assumes (quite correctly) that if you enter more than one address of
a Contact, you might also want to access those addresses.

Outlook for Mac OSX has a significantly superior interface. Firstly, as you
would want, only one contact shows up, then when you select that contact,
multiple possible addresses appear with the default address preselected. It
works on a menu system that folds out to the right. Quite elegant, very
intuitive and easy to navigate. I can't understand why the Windows version is
so inferior.
