No form when using new in business contact

When click on new for new business contact, no form appears; instead, "the
custom form could not be opened. Outlook will use an Outlook form instead.
The form used to view this message cannot be displayed. Contact your
administrator." This is followed by, Array Indes out of bounds. System then
defaults to the Outlook new contact form. Please note, I did not create
any forms. It is the original business contact form in business contact
manager that does not function or gives this message. Thanks.
