new contact form damaged/unusable

OL2003 on xp pro

Whenever I try to crete a new contact, the form is damaged corrupt and
unusable. the text is all over the place and the frames are part there and
part missing, it is totally impossible to use the new contact form. Also,
contacts that are already in my address book only show part of the text. Is
there any way of repairing this? Is it an office problem or an update
problem?
I have run the repair tool twice, to no effect and I have completely
uninstalled and reinstalled the software, again to no effect. I hope I dont
have to do a repair xp install.
