Don't even think of using a contract form that doesn't match your exact
situation. The laws regarding such contracts vary from one state to another,
and may vary with the identities of the people agreeing to it.
For example, the laws of the state of Indiana require certain specific
wording in order for the contract to be enforceable -- see

have other requirements. If you make a mistake, and something goes wrong
that requires taking the parties into court, you may find that a
do-it-yourself contract is worthless.

This is one of many cases in which the advice of a local attorney is worth a
reasonable fee, to avoid large losses later. Quoting
