You have 2 sets of form tags. The second is slightly above the ending <Body>
tag. This one is set for email and local file. Get rid of this second set.
The form properties for the frst set of tags (the one to keep) needs the
email address added as well.

From the look of the HTML, the FrontPage Server Extensions (FPSE) either
aren't installed or have become corrupt.

Try resetting them or ask your host to do this and republish.

Kevin asked how you published. If you used FTP, then this in itself is
enough to corrupt the extensions.

I have tried your suggestions and contacted the company that hosts the site -
they say that Frontpage extensions have been enabled. Anyother ideas? you
can check and see the changes I made.

Kinda doesn't look like FP extensions are installed.

Also, I wonder about this path:
http://www.kingstreetarts.ca/index.htm/on-lineorders.html

I don't believe I've ever seen one like that...index.htm/xxxx.html - I'd
have to defer to one of the super gurus on that though.

How exactly did you publish your site?

To reiterate, if you have used FTP at all, this could corrupt the
extensions. That means they would need to be reinstalled.

The HTML doesn't lie.

If you view the source of a working FrontPage form, you will notice the
shtml reference contained within the form.

When the extensions aren't present, you will instead see the
_derived/nortbots reference. Your source is displaying _derived/nortbots.
This will be a non working or mal functioning form.

Honestly, the extensions can become corrupt as a result of FTP, server
resets or server burps. The point is, when they become corrupt, they need to
be reinstalled.

I use Frontpage to edit my site then I use WSFTP-pro to transfer the file to
the site. Would it work better if I publish right from Frontpage? Also how
do I rest the extensions.

There are a number of FrontPage components which require the FPSE e.g. Forms,
Guestbook, Search Form, Hit Counter etc..

When you use FTP to transfer your component pages, they will not work and
you corrupt the extensions in the process. You said the host enabled the
extensions, but you inadvertently corrupted them. You will need to reinstall
the extensions and then republish with the FrontPage "Publish Web". This is
found under File.

These components require the same as the FrontPage Form Requirements

The FrontPage form relies on three primary ingredients when using the Send
to: option for email and file. This applies to Guestbook, Search Form,
Discussion Web etc.

1. The FPSE (FrontPage Server Extensions) must be enabled on the server.

2. You must publish with FrontPage to http location. Don't use FTP as this
can break or corrupt the extensions.

3. Use of .htm or .html page extension.

Additionally, in order to send results to email, the SMTP (Simple Mail
Transport Protocol) must be set up on the server to send form mail.

If you wish to use the extensions, then you MUST use FrontPage to
HTML publish to the host server.
File->Publish web/site and the location must be in the form
http://www.example.com
Either use the website control panel to reset the extensions, or ask
your host.
Do NOT use WSFTP-pro to upload pages or files to an extended web - FTP
will corrupt the extensions.
