My CheckBox buttons on my form doesn't show when I sent the form

I have created a form some check boxes and options buttons in it. It shows
that I have chosen them but if I mail the form to myself, the buttons doesn't
show that it has been selected.
 
Did you tell Outlook what fields you want to save the data in? Do this on
the Properties dialog of each control, on the Value tab, at the top where
you choose a field.

Have you bound the option buttons to a field, and included code in the item_open
event to initial the buttons from the field?
