I know just enough to make a good add-in, but I've never gone down the
ecommerce road....
1) Can anyone recommend a software-selling service?
2) Recommendation for installation program (e.g., InstallShield?)
3) How do you make it expire after 30 days?
4) How do you setup license codes?

Depends on how you want it work. Common approach store information in the
registry and check against it. If you use a COM dll or exe you can use
protection software like ActiveKey.

That depends. Most online merchants have their own recommended approach
ShareIt has it's own. You could sign up and look that up.

I'll second Shyam's advice almost to the letter, but add CreateInstall from
http://www.gentee.com/ as another installer worth looking into. It's quite
flexible and costs a good bit less than Installshield.
 
I like the QSetup installer that is free to independent
developers:
http://www.pantaray.com

Very nice, very flexible and it has the expiration and
licensing feature built-in.


For distribution; if it's good, you can get on
download.com (upload.com) or tucows.com
