Using MS Optical Intellimouse and Ver 4x Intellipoint driver. Works great
most of time but lately, every so often I'll loose the arrow. It's like
it's still there, but I can't see it. If I move the mouse around, I can
tell when it gets over a menu button or icon etc, I just can't see the
arrow. If I "alt-tab" to another application, the arrow is usually back
but when I "alt-tab" back again it's still gone. If I close the app and
restart it, the mouse is fine again.

I don't know if this will work for you but it did for me.

Open the software, the mouse properties from the start menu > programs >
microsoft mouse.

Click on pointer options. Untick hide pointer arrow while typing. Untick the
pointer trails if you don't need it. Slow it down a little if you can - the
horizontal scrolling seems to be a bit buggy at times as well.

Barring that check your processes and see if it's eating a number of undue
MB of RAM or system resources or if there's another application doing so at
this time.
