moving excell form to access

im new to access...i need to collect names, dates, hours and rates on one
sheet and be able to move it a standardized form i have created in
excel...when i try to paste the form from excel to access, my whole form
changes and i need the form to stay as is

Access is not "Excel on Steroids". It's a different program, with different
structure, different design, and - important for your case - different Forms.
An Excel form will not work in Access, or vice versa; I think you'll need to
recreate the form to look and work like the Excel form.
