Moving Custom Forms from Exchange 2000 to Exch 2003

I just migrated a company from Exchange 2000 to Exchange 2003 but they have
several custom forms that I need to move. Is there a way to export or move
these, and the source code, to the new platform?
 
You could run each form and save it as an loft template file, then republish it.
 
First, there is no difference in how custom forms are implemented, developed
or managed between EX 2000 and 2003.

Your best bet is to save the form definition from every library that has a
published form as an .fdm file, and publish back to the same location in the
new Exchange deployment using the .fdm file. If you are migrating your
entire Mailbox and Public Folder stores, these published forms may even come
along anyway (don't quote me on that though!).

BTW, the code is stored with the form.

Thanks for your assistance. However, these forms are customized in the
effect that there are server names and network paths inside of these forms.
It references a AccessDB on a folder on a server. That server is going away
so we must edit these files and re-import them. There are three forms all
together. A Request form, Approve form, and deny form. The request makes
the call to Active directory users to send the request to the users manager
for approval or denial. The request form is somewhat working but when we
click on the approve or deny buttons, it doesn't see the other forms
(App/Deny). I need an exchange/vba programmer!!!! AAAHHHH

Any other information would be helpful.
