I've had this same problem repeatedly, on only a few machines. It seems
pretty consistent, that one machine always does it, and another machine
never does it. It also seems like every machine of a particular model (i.e.
Compaq Deskpro) does it, while none of another model (i.e. Dell Optiplex)
have this problem. This leads me to think that it's hardware-related. I've
tested this theory to some extent, by loading DOS 6.22 on these same two
machine models, and still have the same problem. Within Windows 2000,
removing and reinstalling the mouse driver results in a reboot, which makes
the mouse work but is not optimal. Simply scanning for new hardware has no
result.
All in all, it's pretty annoying but not crippling. I only use KVMs on my
file servers, which also run Terminal Services. I can still log in normally
through TS, with no mouse problems, so I just keep the server room locked
and don't worry about it
