Things to try, making only one change at a time, testing after each
change:

1. What changed between the time things worked and the time they didn't?
2. What surface is the mouse on? Optical mice are fussy about the
surface, disliking glass or otherwise shiny surfaces, mousepads that
are patterned or too light, etc.
3. Try a different mouse.
4. Update mouse drivers.
5. Update video card drivers.

I've also noticed that if a hair gets stuck in there, that'll do it too...
Make sure the underside of the mouse is clean.

Cheaper optical mice tend to have that problem, what type of mouse do
you have?

Well the one it usually happened to me on was an MS Intellipoint Optical
Trackball.. Bought it in 2001 ... still use it to this day with my laptop
and occasionally my desktop machine .. Still works great!
