I have reinstalled Outlook 2003 after the trial of Office 2007, now my
send/receive and new message buttons are missing, yet if I press F9 send and
receive for all accounts works fine.

Locate outcmd.dat and rename it to .old to reset your toolbar. This is
recommended when doing a downgrade.

Tried this, restarted Outlook, the send/receive button shows very briefly and
then dissapears again.

Verify that the Toolbar isn't being compressed becaus lack of space for the
toolbars. Rearrange them or press the button at the right-end of the toolbar
to make sure.

Alright, lets take a different approach and recreate the mail profile. This
is actually also a recommended procedure after changing your Outlook
version. For details see;

Looked at the web page you gave the link for, created new account, but when I
went to create new "PST" the system would not let me click in the box to name
it.

Which box? To name the pst-file or the display name for Outlook? The latter
can only be changed in the Advanced... dialog.
