I have a couple of reaccuring problem.
1. I run the antispyware and it detects TV Media. I confirm the entrys as
reported using regedit. I let the software fix the problem and then confirm
the fix using regedit.
2. I then run a program called Autoruns. It shows
"HKLM\SOFTWARE\Microsoft\Windows NT\CurrentVersion\Windows\Appinit_Dlls
mad.dll" which is associated with TV media according to a google search on
the mad.dll.
I delete this reference from the registery using the Autoruns software and
confirm the deletion using regedit.
Then I try to delete the mad.dll from C:\winnt\system32 directory, windows
reports that the file can not be deleted.
I've tried this course of action in normal Windows mode and in Safe Mode,
both results are the same.

The questions I have are: How can I determine what is actually causing the
TV Media to be reinstalled or reactivated and how can I determine what keeps
putting the mad.dll back into the registery?

Try using Start > Run > cmd then type in regsvr32 /u
c:\winnt\system32\mad.dll when the command prompt comes up. Now you should
safely delete it from explorer. The reason that you couldn't remove the file
is it is a registered component of TV Media. The problem is, since it's a
registered component, TV Media likely wasn't removed when you had MSAS try to
remove it. Hopefully you can remove still remove it since you removed some
registry keys. Hopefully you made a backup of the registry before you made
these changes, otherwise you might have to manually remove the files from
your system.
