making a macro for a 'check ' box

Hi I want to place an automated check box in a word document like those in
the word templates for faxes. I need to be able to check the box withh the
space bar. Can anyone help. I tried copying it from the word template but
that was unsuccessful.

If you want to check it with the space bar, then you need a check box form
field. If you will settle for a double click, see
