Microsoft's Help system has landed you in the wrong newsgroup: this
one is for the database program Microsoft Access.
I doubt anyone has a specific template for this; you might want to
talk to whomever is handling your bankruptcy. If you're doing it
yourself... well, remember what they say about a lawyer who defends
himself: he has a fool for an attorney. See if there are any
public-service legal advisers who can help you.
