Lead-in paragraph header in document map and pdf

In word 2007, I can easily format the lead-in words to a paragraph as
heading, so that they show up in the ToC. However, I've had no luck in
getting them to easily show up in the document map or, for that matter, in
the automatically created bookmarks upon conversion to adobe pdf. I suppose
for the latter, I could manually create bookmarks, but I'm trying to
efficiently use headings for the outlining tool, and convince others to do
so. Any tips on easily getting these headings into the document map?

If you use the Style Separator feature, you'll find that run-in sideheads will be included in the Document Map and they will also appear as headings when you convert documents to PDF (using Word's Save As PDF feature at least).
