This has happened before, but I don't remember the cause or the fix:

When using right-click \ Save Picture As on a jpg file, I'm presented
not with the name of the file in its native format, but "untitled" and
only as a bmp file.

To my knowledge, the only thing I've done to my system that might have
any impact is I applied a few Windows Updates last night:

Recommended Update for Windows XP SP1 (KB822603)
Update for Microsoft Windows XP (KB826942)
Cumulative Security Update for Internet Explorer 6 SP1 (KB824145)

XP Pro sp1, IE6.0.2800.1106.xpsp2, Update Versions says "SP1"

How do I get MSIE to not change the file name and details on images I
want to save that I'm viewing with it?

It's time to perform some simple housecleaning chores:

Go to Start > Run and type: CLEANMGR.EXE , and hit enter.
This will start your disk cleanup utility. Make sure to cleanup
your Temporary Internet Files.

That does it, thanks. This time, I'm saving this information.

So far as I can tell, this is a sizeable bug in IE if either the size
of the local cache or an "unknown" or "damaged" activex control can
break this.

I will plan in advance what to do with each of my enemies if they are
captured. That way, I will never have to order someone to be tied up
while I decide his fate.

Jeffrey;
Consider this:
Right click the IE icon | Properties | Advanced tab

Scroll down to and make sure this is checked:
Empty Temporary Internet Files folder when browser is closed
[Specifies whether to clear the Temporary Internet Files folder when you close
the browser.]
Apply | OK
