Item in sent items but not sent

Weird problem and I was wondering if anyone else has come across it before?

A user mentioned that an email she send didn't get to it's recipient, this
was strange as it was an internal email and the email was showing up in her
sent items. Strange thing was, when we brought up the "From" field in the
sent items all emails had the users name but this problem email had none.
Looking further down the list there were a couple more that had no name
against the "from" field and it looked like they had not been received
either.

I've tried reproducing but can't seem to. Has anyone any ideas?

Outlook 2003 SP2
Exchange server 2003 SP2

Looking at my own sent items, I seem to have a couple with no from details
but they're a while back and I can't be sure if they reached the recepiant
or now. I'm on Outlook 2007.

You may want to enable tracking in Exchange. Are there any header details at
all?
 
Message tracking shows up nothing for the particular emails, also the user
on has one a/c and email address.
