Inserting PDF form

I'm trying to add a job application form to our website. I'm not sure how
safe it would be to have someone fill out an application and email it back.
So I'm just going to provide an application that people can print off. I
have saved the Word document as a PDF. I've imported it (don't really know
where it's at after I do this).

I'd like the application to be clutter free of url and date, so I thought a
PDF would eliminate the titles and the stuff on the bottom of a commonly
printed web page.

Can you help?
 
Simply put the PDF in any web directory and create a link to it on your web
page. Browser plugins will handle the rest. You may or may not wish to
include a link to download Adobe Reader.
 
By web directory, do you mean the folder that says "Web sites"? I've done
that it doesn't work. So I'm guessing that I'm wrong on the destination. So
can you give me an example of where I could find a web directory?
 
The file can be anywhere in your web, putting in the root directory or
"images" folder would be fine.
