Inserting checkboxes on a user form

I have created a user form that copies the information entered onto the word
document it is attached to. I can't however copy over checkboxes on the user
form to check boxes in the document. Does anyone have an idea where I am
going wrong?

Insert the appropriate symbol in the required place in document, based on
the status of the check box in the userform.
