Inserting bookmarks

How can I insert a bookmark in a document ofr the purose of a cross reference?

I go to Insert --> Bookmark

But when I enter a bookmark the "Add" option is blanked out, and the only
thing I can do is close it.

What is the use of the "Add" option if you8 can't click on it?

How do you unblank it?
 
You need to put in a valid bookmark name before you can ADD. Most likely is
you are not using correct bookmark name.

Bookmark names cannot contain spaces. Most likely you'll see the Add button
suddenly dim as soon as you type a space.
