incident report for security service

small security service requires a template of an incident report type of
incident gaurd on duty discription of offender
 
Do a hunt for Record Templates for Access and see if any of those can be
adapted meet your needs. If your needs are fairly specific, then you will
either need to spend some serious time learning how to create your own or
pay someone to do it for you. Having said this, if you already have some
knowledge of Access and have the time to spare, then it sounds like an
interesting project for a beginner.
