In addition to what Russ posted - create a new contact in Outlook, and
export that to Excel. Check that your created Excel addressbook fields match
the Outlook ones. then in Excel name the range of data before you try the
import.

Would suggest that anyone who is importing any data into Outlook, regardless
of input file type, <ALWAYS> go through the "Map Custom Fields" step
whether or not the column names match those of Outlook. Getting used to
letting Outlook "auto-map" field names (which basically is the only benefit
to changing column names) without manually checking the mapping, can easily
lead to problems (missed fields because of column name not changed correctly
etc). Also, by going through that step, it negates the need to rename column
headers altogether.

true - but that process can be quite un-intuitive if you are not familiar
with it.
