Icons and taskbar have disappeared - can't even right click on destop

Have a machine that rebooted one day and when it came back up, it only shows a blue screen with the mouse pointer. No Start button, no taskbar, no time etc.. When I right click on the desktop, it doesn't give me a menu. The only thing it WILL do is alt-ctrl-del the task manager comes up and I can run things from the File>New Task prompt. When I restart, I see a glimpse of the icons and taskbar before it shuts down.

I have tried to reboot in safe mode, everything looks normal. I have created another user with admin privs and it has the same problem. I have also run a full symantec virus scan and everything is clean.
 
This is the consequence of a damaged Windows Explorer (explorer.exe).
To correct the fault, Explorer needs to be replaced. If turning off the
..exe file via the Task Manager and turning it back on does not work,
a new copy needs to be taken from the i386 folder. For details, please
