I asked a similar question a few weeks ago and iconforge was suggested. I

I've checked IconForge but it seems to be payware.

British mags make certain payware apps freeware;
This one is courtesy of PCPlus

IconForge v4.92 A comprehensive graphics utility
for creating and editing Windows format cursors, icons,
multi-resolution icons, animated icons/cursors, and bitmap
images. Its tools include: brush, line, circle, rectangle, flood,
eraser, spatter, rotate, flip, area selection, text, nudge, zoom
and more. Transparent and inverse video effect areas may be
easily painted onto images to allow backgrounds to show through.
Edit with multiple icons at the same time for easy cut and paste
between images. Special effects filters help to make painting your
own icon-sized pictures a snap, and common animation effects
may be produced automatically!
