I send an image via Outlook it often does not reach the receiver

When I send a picture in an email my receiver complains that they do not get
a picture is this my fault or theirs?

Hard to say without any configuration information.
Verify that you are not using Rich Text but use HTML or Plain Text instead.
In which message format does the receiver get your message?
