I need a form for personal information on employees

I need a form that will allow me to gather & keep personal information on
employees, to include name, telephone number, address, all info pertinent to
a person.
 
If you're planning to do this using Access, I beg to differ. First you need
a database design. When you've got that right, then it will be time to start
thinking about forms.

Look at the employee table and form in the sample "Northwind" database that
come with the Access package. That should steer you in the right direction.
