I can't make a Form in access

please can some one help me I can't make the form in acces ??

First I made a table than when I try to make a form I can't the problem that
write in the window is : The wizards was unable to generate fields form the
record scoure you choose. Perhaps you chose a query that doesn't return any
fields.
 
What record source did you select in the first window that pops up after you
clicked the NEW button?
Does that record source contain fields?
