I am trying to make a application page for a dog rescue

I want to make a application page using front page. I would like to know how
to make this page, and then have a submit button at the bottom of this page
to send me the answers of this application.

Please help, as this is for a dog rescue web site

If your hosted site has the FP SE and email transport enabled use a FP form
- File new page and try the forms Wizard
