Hello
How to stop Outlook 2003 sp2 showing "Are you sure you want to permanently
delete all the items and subfolders in the "Deleted" folder" every time I
close Outlook?

Either turn off the setting to empty the Deleted Items folder on exit or turn off the setting that prompts the user when items are deleted permanently. Both are found in the tools | Options dialog.

tools, options, other... empty deleted on exit and/or on the advanced
button, warn before deleting.

Will unchecking "warn before deleting" prevent the warning to delete items
in all folders during Outlook usage, not only items in the "deleted items"
folder at Outlook closing?

it affects all deletions- not just from the deleted folder. Typically, it's
not a problem as the deleted items go to the deleted folder and you can
rescue them... as long as you don't delete on exit. I personally prefer to
use autoarchive to clear it out every couple of days - outlook closes much
faster this way too. You could set if to run daily and delete all mail 1 day
old - by I like to keep the deleted items for a couple of days as insurance
for accidental deletions.
