When the list of Temporary Internet Files are viewed from "My Computer",
it shows not only the date/time that the item was cached but also
the URL where it supposedly came from.

Where is this information (especially about the URL) actually kept?

Is there a way to save or capture this list of details to a text file
directly or indirectly?

Have a look in Internet Explorer, History. You'll find the URL & date last
visited under Properties.

In an index.dat file, located here :

\documents and settings\username\local settings\temporary internet files\Content.IE5
