I accidentally deleted the switchboard form and cannot reinstall it.

I assumed this is the switchboard created by the built-in Switchboard
Manager.
Change the name of the Switchboard Items table to Switchboard
ItemsOld. Then create a new switchboard using the Manager. When done
save the new switchboard. Then Delete the new Switchboard Items table.
Rename the Switchboard Items Old to Switchboard Items.

I forgot to say that all you need do is add one item to the new
switchboard, not all the items in the original one.
