How does one prevent all items from being added to My
Recent Documents folder? Is there any reason why this
would be a bad idea?

Right click start and select properties|customize|advanced. Uncheck 'list my
most recently opened documents'.

You can prevent Windows XP from listing your most recently opened documents within My Recent Documents by doing the following.
Click the Start button, click Control Panel, click Taskbar and Start Menu
Select the Start Menu tab and click the radial button beside Start Menu
Now click Customize... and then click the Advanced tab
Uncheck the 'List my most recently opened documents' checkbox
The My Recent Documents menu should no longer appear.

Open Regedit.exe and browse to this key:
[HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Policies\Explorer]
In the right-pane, set the "NoRecentDocsHistory" to 1 and restart windows.

This will prevent Windows from adding the recent files to the list [and also the 'Recent' folder]

Neither suggestion seems applicable.
'List my most recently opened documents' is unchecked
already, and the Registry key "NoRecentDocsHistory" does
not exist. Any other ideas?

Get TweakUI for XP...it allows you to allow many aspects of Windows that
you would otherwise nopt have easy access to including taskbar/start
button tweaks.

You'll need to create the key. If the key isn't there,
WinXP uses it's internal default - NoRecentDocsHistory = 0
where 0 means FALSE.
