How to enter a field in Microsoft Word 2000

Please help.

I have created a document and want to use it as a form
where people can enter information in certian fields. For
example, Your Name XXXXXXXXXXXXXXXXX, YES or NO, Date,
etc....

How can I enter field where people can just type in the
information and not type over the question? In pdf files
there are "fill in" fields and people can just tab over to
the next question after completing it. Is this called a
template?
