how to edit a prepared application form with access

My business uses a form for every client. I would like to scan it into my
computer and then be able to enter names addresses so forth, on screen and
print it out. it also has boxes that require calculations like inserting a
date 15 days from today's date. I would like to be able to have those
calculations performed too. Can anyone point me to an online tutorial? Or
know where i should go to look for information on how to set this up?

one way of doing it is to save the file as a jpg or whatever and then
use it as a background for your real form and then drop controls on
top of the textboxes or whatever. Not pretty, but might be your best
bet.

Thanks I'll try this first. I know I have seen this in an Access or VB text
book at some time, but I'm away from my reference books and I can't find a
tutorial on this. There must be a name for it, but I can't think of it, I
don't even know what to search under.
Any other suggestions ? Anyone? Anyone? :) Thanks, C
