how to design forms in outlook

If a manager apporves a credit on a csutomer's account, I need a form in
Outlook that the employee can fill in the amolunt of the credit, the account
number and the Manager' email address (from a distribution list) to send a
reminder to the Manager's inbox so they will access the customer's acct to
notate their approval of the credit. Is this possible? How does one do this?

Assuming you are on an Exchange system, you need to investigate the workflow and
routing capability that is built into Exchange, as it is specifically designed
for just this scenario.
