How to creat forms in Word?

Hi,

We need some custom forms for our office. Nothing fancy, just simple one or
two page forms with some labels, text areas and check boxes.

We'd like to be able to make these forms (templates?) available to our
employees and have them fill them out onscreen and save them in a special
folder. The thing is, we want them to be able to "fill them out" but not be
able to edit the formatting, labels, design elements, etc.

Is this something we can do with Word, and if so could anyone offer a little
direction or maybe point me to some online tutorials?
