How to "automatically" insert last name after Dear Mr xxxxx?

many times I am creating letters, which are the same contents in the letter
with small differences. Once example would be all that I need to change is
the name and address in top of the letter, with the person's last name to
default into the Dear Mr/Mrs/Dr. etc.

The list these names are pulled do not come from a database but merely
periodically I am asked to send to these people when they received awards or
publish or are recognized publically. Of course, the date on the letter has
been set to default to the system date, now I would like to "automate" a bit
more.

You can do a mailmerge, but the names will need to be in some sort of
structured format.

Where is the list of names?? What format is it in, or is it just typed on a
sheet of paper...?
 
Create a template letter with all the common text and use the CreateDate
field. When you open a document from the template, the date will update
itself to the date you create the new document from it (and it will be
locked).

However, you cannot automate the entry of the name and address if they are
not contained in a database or contacts lists or something similar. Where
are these names and addresses held?
