How Specify Category and Subcategory for Outlook Templates

I have a number of OFT (Outlook Template) files saved in a folder. To
use one of them in Outlook 2003, I navigate to File Menu => New =>
Choose Form. In the Choose Form dialog, in the Look In drop down I
specify "User Templates in File System" and Browse to the appropriate
folder. When viewing in this dialog the list of templates in a tree
view (via the right-most button at the top of the dialog), it appears
there's a way to assign a Category and Subcategory to the templates.
All of them are subordinate to a node labeled "<No Subcategory Name>",
which is itself subordinate to another node labeled "<No Category
Name>".

How do I specify the Category Name and Subcategory Name for my Outlook
templates?
