How set default Flag Status to "cleared" in custom form

Somehow i inadvertently set the Flag Status on a custom form. Thus, all new contacts entered with the new form have their Follow Up Date set to Sept 21, 2012. I have tried everything but am not able to get new contacts to have a "cleared" flag. Any ideas?
 
ok, just figured it out. apparently before clicking on the "Developer" button and "Design This Form", you can click on the "Follow Up" flag in the header and clear the flag. Then proceed with Designing and then Publish and the flag will be cleared by default.
