The birthday Field is on the 2nd Tab of each Contact, the Details Tab. Set
a date there and it creates a recurring event with your time set to Free and
reminder of 18 hours ( I think that's the default).

if you have birthdays on thecontacts, outlookshould create the event on
yourcalendarand if you have reminders enabled by default, it will set a
reminder.
if you deleted them or importedcontacts, you'll need Put Anniversary
(http://www.rsoutlook.com/us/prods/prod05.html) to create the
appointments.

If I have imported thousands of contacts into Outlook, it does not
automatically create the birthday reminders. Is there a way to make
this happen without going through each contact one by one?

Thanks!
