how do you create an index if you've manually created all the entr

What if you've already created the index manually. I'd like to know how to
make the dotted line leading from the index item to the page number---how do
you do this so the number right-aligns? Do you have to put the dotted line in
manually for each item?

Why in the world have you created an index (or even a TOC, if that's what
you really mean) manually? But what you need is a right-aligned tab with a
