How do I Use forms in Outlook 98?

Does anybody know how to get around a problem of automatically adding a subject to an e-mail in Outlook 98? My organisation currently uses both Outlook 98 and 2000 and is trying to find a way to automatically add a phrase to the subject of particular e-mails. The problem was solved in Outlook 2000 with the use of a form which added the phrase to the subject, however this option is not available in Outlook 98 and i was wondering if anybody knew how to implement forms in 98 or if there was another awy round the problem?
 
Forms should work in Outlook 98 as they do in 2000. What particular issues are you seeing?
