The answer is....

You must remove your personal calendar. You will see, however, that
you can't simply right click and delete the calendar. Instead, you
must completely remove the personal folder file (stored as a pst file).
This will remove your local folders that store email, any local
contact folders that you have created, and, importantly, your personal
calendar. You remove this by right clicking on it and choosing to
"close" it or to go to File --> Data file management, select it and
then "Remove". Before you do this, you could create a new personal
folder file and then transfer everything except the calendar to the new
personal folder file. Once you close it, create a new rule or modify
you existing rules to point to you new personal folder file. Now, see
what happens when you accept the next meeting request. You will see
that it goes to the server calendar.

The personal calender most likely showed up because you choose to view
it. Since it didn't exist, Outlook created a new one for you. From
that time on, all accepted meeting requests went to the newly created
personal calendar.
