how do i set up service call register in access database?

we a manufacture of machine and HI-TECH security equipments. we want to
maintain a service register to main our service calls received from the
customer end. that report should contains the following.

serial no.
date of call received
name of customer
product in used in their end.
installed location
date of installed,
product have warrenty period
nature of problem
soultion for problem
action taken
present status of problem
any spares
remarks from customer end.
remarks from factory end.
commants
spares of warrenty or cost.

You never really learned. These newsgroups are for FREE support, not an
