How do I send an outlook form as a message?

I ahve created a form in Outlook. I actually modified the message form and
created a form to send to a client. This form will enable the client to fill
in needed information and send back to me. I successfully created the form
and can datafill the necessary information. I then sent it to myself to see
if I can receive it, however what I actually get is just a blank message
form. Is there some permissions that need to be set?
 
I'd suggest asking this in a newsgroup related to Outlook.

This newsgroup is for questions about forms in Access, the database product
that's part of Office Professional.

I would suggest that you ask this question in an Outlook newsgroup.
This newsgroup is for Microsoft Access forms, which are quite
different objects from Outlook forms.

My *GUESS* is that an Outlook Form (like an Access form) is internal
to Outlook, and has no independent existance; but the volunteers over
on microsoft.public.outlook.forms will have better knowledge than
mine!
