You have several options. If the form is available in an electronic
format, you can open the form, hold down the Alt key, and hit the Print
Screen key on your keyboard. This will copy a picture of the frontmost
window on the screen. You can then either
* go to picture editing software (such as Adobe Photoshop), paste the
file into a new document, edit it there, and save it as a JPG file to
insert into your PowerPoint (Insert menu > Picture > From File)
* go directly to PowerPoint and paste the picture right onto a slide.
If you don't have an electronic version, you will need to scan in the
form with a scanner. Then you can save it as a JPG file and insert it
into PowerPoint.
