HOW DO I MAKE A DAILY COLLECTION WORKSHEET FOR MY OFFICE?

I am trying to make a collection worksheet for my office. It needs to include
the following:

Patient Name
Physician
Cash
Check #
Credit Card
Total Payment
Daily Totals

How can I do this?
 
This is a little like asking "How do I fly a jumbo jet." You start by
getting in the cockpit and operating the controls.

You obviously know how to use a keyboard to combine letters into words.
What specifically do you need assistance with?

I don't think Greg was trying to make a 'Smart Remark', I think if you look
at his other posts, you'll see he is trying to HELP you.

He was just saying that if you'd like some help, you really need to be a
little(lot?) more specific as to exactly what your requirements are...
Is it just a form you want to print out, or do you need some way of storing
the information collected(like a database)?
Do you require it to be filled in on-screen, or just written on by hand?
etc.
etc.

The offending remarks were intended only to provoke you into asking a
question that someone could have a reasonable chance of providing a
thoughful and meaningful answer.

If you want someone to design a form for you there are certainly many
people willing to assist. Some for free, some for a fee. Either way,
you are going to have to provide specific details.

Actually JoAnn they ask me why I'm there to see the doctor and I tell them
because I had nothing better to do than to complain about what bothers me
and the doctor is the only one who will listen.... :)
